During your first visit to his Chicago office, Dr. Shastri will start by asking what your goals are for your nose. Following a thorough analysis of your nasal anatomy, he will discuss how he may help realize your goals while still preserving a natural appearance and maximizing nasal breathing. Image modification will be done in real-time with your photographs to help make sure you are both on the same page about a realistic outcome. Dr. Shastri will take the time to make sure you feel heard and confident about the surgical plan.

What to Expect on the Day of Rhinoplasty Surgery

In most cases, rhinoplasty is performed as an outpatient procedure under general anesthesia. Surgery may take 3-6 hours (sometimes longer in complex or revision cases). Dr. Shastri carefully chooses the approach based on your unique needs. In open rhinoplasty, a small incision is made across the columella (the tissue between the nostrils), allowing for complete visibility and access to the nasal structures. In closed rhinoplasty, all incisions are hidden inside the nose, offering a more discreet approach with minimal external scarring. As discussed during the consultation, both techniques allow Dr. Shastri to precisely reshape the nasal bones and cartilage to achieve the desired aesthetic results.

To begin the rhinoplasty procedure, Dr. Shastri will use well-hidden incisions along the base or the inside of the nose to access the underlying bone and cartilage. He will reshape the nasal bones and cartilage according to the plan discussed during the consultation. The septum is corrected and straightened in 100% of cases; cartilage from the septum is used to structurally redefine the shape and strength of the nose. In cases where septal cartilage is absent or too twisted for use, Dr. Shastri may need to obtain more cartilage from an ear or rib to achieve the intended goals fully.

At the end of the procedure, the incisions are carefully closed, and soft nasal splints are placed inside the nose to keep the septum straight. Tape and an external cast are applied to the outside of the nose. These splints and external stitches are removed 1 week after surgery. Patients typically spend 1-2 hours in a recovery room after surgery and can be taken home afterward by a friend, helper, or family member.

Rhinoplasty Recovery

Recovery varies from person to person and depends on how much work is needed, but generally, healing can be thought of in three phases:

  • Phase 1: For the first week after surgery, there are thin/flexible plastic splints within the nose and a cast over the top of the nose. Swelling and bruising are expected, and patients are recommended to use nasal saline sprays regularly to keep the nasal passages clean. Patients are encouraged to take the first week for rest and recovery at home and typically can return to work after the first week postoperative appointment.
  • Phase 2: Gradually, between weeks 2 and 6 after surgery, swelling and bruising improve; any residual bruising can be easily concealed with makeup during this time. Patients may start to resume exercise and heavy lifting by 3 to 4 weeks after surgery. By week 6, approximately 75-80% of swelling has resolved.
  • Phase 3: After 6 weeks, the majority of swelling from a rhinoplasty has improved, and the nose continues to heal and settle for several months. Results are typically not considered final until 1 year after surgery, but small/incremental changes may continue to be seen for up to 2-3 years, depending on the thickness of the nasal skin.
